Intake Form (Elation)
You will enter your symptoms, medical history, and current medications in our intake form. This is hosted on Elation, a HIPAA-compliant electronic health record system.

You will be redirected to Elation's secure site. While outside of Sky e-Clinic's domain, your personal and medical information is strictly protected under U.S. HIPAA standards. Please complete the form with confidence.
Online Consultation

A consultation link will be sent to your registered email before your appointment. Click the link to start your video or phone visit instantly.

Send your message anytime — 24/7. The physician will review and respond as soon as possible.
🔒 HIPAA-Protected Communication
All video, phone, and chat communications are encrypted using HIPAA-aligned systems. Your information is not disclosed to third parties without your consent.
Electronic Prescription
When a prescription is needed, the physician sends an e-prescription directly to your pharmacy. No paper prescription required.
📍 Please confirm your pharmacy details
Enter your preferred pharmacy name and address during intake. Any pharmacy works — CVS, Walgreens, or a local drugstore. Please also check the pharmacy's hours so you can pick up your medication as soon as it's ready.
In-Person Referral if Needed
Based on the consultation, you may be advised to:
- →Visit an ER immediately (if symptoms turn out to be more serious)
- →Upgrade from a chat visit to video or phone for a more thorough evaluation
- →See a specialist or in-person provider for care that cannot be completed online

We hear you. But issuing medication without a proper examination is irresponsible and puts your safety at risk. At Sky e-Clinic, patient safety always comes first.
At the same time, as an active ER physician, I can help you think through whether an ER visit is truly needed. When the situation warrants emergency care, I will say so clearly; when it likely does not, I will explain my reasoning.
